Absolutely. We highly encourage visits. This allows you to see the facilities, meet some of our team, and get a feel for the environment.

Yes, our services offer a diverse programme of activities designed to promote physical, mental, and social wellbeing. This can include gentle exercise, arts and crafts, music, games, gardening, reminiscence therapy, and organised outings (where appropriate). We encourage individuals to suggest activities they enjoy.

We encourage individuals to personalise their rooms to make them feel like home. Within reasonable limits and subject to safety regulations, personal belongings, photographs, and small items of furniture are welcome. Please discuss this with the service manager prior to admission.

Our teams provide nutritious and varied menus. We can accommodate a wide range of dietary requirements, allergies, and preferences. Please inform us of any specific needs during the assessment phase.

Smooth Transitions: Cygnet’s Co-Produced Process

Cygnet’s Co-Produced Transition Process is a person-centred approach that ensures individuals, their families, and multidisciplinary teams work collaboratively to create a smooth and supportive transition plan.

By actively involving each person in decision-making, the process promotes empowerment, continuity of care, and tailored support to meet their specific needs, helping them achieve positive outcomes.

Key individuals involved include the person being referred to Cygnet Social Care, their social worker or case manager, Cygnet stakeholders, familyor carers, school, and internal or external
Multidisciplinary Teams (MDT).

The transition involves several important steps, including agreeing on timelines, making bespoke environmental changes and decorating personal areas, and an internal referral to Cygnet’s physical health and Positive Behaviour Support (PBS) team for initial screening.

We also ensure registration with a local GP, undertake MDT assessments where required, complete health action plans and hospital passports, and send an easy-read welcome guide to the individual.

Additionally, a carers handbook is sent to the family, legal frameworks like Mental Capacity Act (MCA) and Deprivation of Liberty Safeguards (DOLS) are formulated, an activity planner is completed, and medication management is handed over by admission day.

Communication and Visiting

We believe in open and regular communication. You will have a named key worker or primary contact person who will be your main point of contact.

We provide regular updates on your loved one’s health, support, and overall wellbeing through preferred methods of communication (phone calls, emails, or meetings).

Absolutely. Your input is invaluable. We actively encourage families and carers to participate in planning meetings and reviews.

We value your insights into your loved one’s history, preferences, and needs.

We operate an open-door policy where possible and encourage families and friends to visit regularly. We understand the importance of maintaining strong family connections. While there are generally no strict visiting hours, we kindly ask visitors to be mindful of individuals’ routines, particularly meal times and rest periods. Please check with the specific service for any current local guidelines or restrictions.

Financial Matters and Regulation

Fees are determined by the level of support required, the type of room, and the specific services provided. During the assessment process, we will provide a clear breakdown of the costs to the funding authority.

Services are funded by local authorities and NHS Integrated Care Boards (ICB).

All Cygnet residential services are regulated by independent bodies.
