Cygnet Health Care offers a PICU / Acute transitional service (Discharge 2 Assess D2A) for people who no longer require an acute admission, but who remain on PICU and Acute wards as a delayed discharge due to not being able or ready to transition into the community.
The service is offered at all of our high support inpatient rehabilitation (Level 2) services within the UK.

Our PICU / Acute Transitional Service (D2A) is suitable for individuals who no longer require an acute admission but:
- Require some support prior to discharge to the community
- Do not require a long term rehabilitation placement
- Need a period of assessment to identify their next placement
- Have a placement identified with no agreed admission date
- Have no fixed abode and require housing / benefit support
